NYS Exam OPT IN/OUT Google Form

Dear Parent/Guardian, As you know we will be administering the 2020-2021 NY State ELA, Math, and Science exams in person. Attached to this email you will find a google form that allows you to check off the state exams you would like your child to be administered or the option to OPT your child out of the state exams. Please complete the form no later than April 7th, 2021. Just a reminder, if your student is taking the Regent Exams they must also take all State Exams. CLICK ON "Read Full Story" TO GET THE FORM. URGENT! Deadline Thursday February4th Mandatory COVID-19 Consent Form

ATTENTION: As per the Chancellor's directive, you MUST complete a COVID-19 Consent Form by 3:00pm, Thursday February 4th, 2021 in order for your blended learning student to return to the school building. 8th Grade Memory Book

8th Graders - Please help us create a Memory Book for Class of 2021. For now, please use the code nqcukpz to join on Google Classroom. We will be asking you to add photos and add short responses to specific questions at some point. This will be an important memory in your future-life, but we can't do it without your participation.

If you want to change your student's instructional model to remote learning (100% at home), please fill in this form. Please understand that this is not immediate. It takes at least 3-4 days to occur - have your student continue with their current classes until you receive notification of your child's new class and schedule.

Learning Bridges Program - help with child care options

The DOE and the Department of Youth and Community Development (DYCD) are collaborating to launch Learning Bridges - a new program that will provide free child care options for children from 3-K through 8th grade from 8am -3pm on days when they are scheduled for remote learning. Family Income Inquiry Form

Dear Families, As you may know, we ask families to fill out a Family Income Inquiry Form each school year. All New York City public school students receive free lunch. However, the Family Income Inquiry Form is a key factor in determining whether your child's school is eligible to receive Title 1 and other funding. For example, Title 1 provides resources for programs and services to eligible schools. By submitting this form, you can help ensure that funding for your child's school is properly calculated for these resources. We rely on Title 1 money to help with some of our programs. Please complete the Family Income Inquiry Form online at by October 20, 2020. If you receive benefits from Supplemental Nutrition Assistance Program (SNAP) or Temporary Assistance to Needy Families (TANF), your child's school is eigible for additional funding.
